Protocolo de actuación ante dos situaciones en osteoporosis frecuentes en Atención Primaria: cuándo tratar siempre y cuándo evitar un tratamiento innecesario

Abstract

La osteoporosis es una enfermedad a menudo infradiagnosticada. Los pacientes fracturados son los que tienen más riesgo de sufrir una nueva fractura y en ellos debe centrarse el cribaje para diagnosticar esta enfermedad e iniciar de inmediato un tratamiento. En el otro extremo, en la mujer postmenopáusica reciente, a menudo se indican tratamientos para la osteoporosis ante la simple presencia de algias difusas y el temor de la paciente a tener la enfermedad. En estas pacientes es aconsejable tratar de cuantificar el riesgo de fractura y evitar en lo posible la prescripción de tratamientos farmacológicos.

Osteoporosis is a disease often underdiagnosed. Fractured patients are those with higher risk of a new fracture and they should focus on screening to diagnose this disease and begin treatment immediately. At the other extreme, in recent postmenopausal women, often treatments for osteoporosis at the mere presence of diffuse pains and fears of the patient having the disease are indicated. In these patients it is advisable to try to quantify the risk of fracture and avoid possible prescription drug treatments.
