Title: Plasmid mediated resistance to trimethoprim in Enterobacteriaceae isolated in Gran Canaria, Canary Islands (Spain)

Abstract: During a 2 month period, 145 Enterobacteriaceae isolated from clinical specimens, at the Hospital Nuestra Senora del Pino (Gran Canaria), were tested for their susceptibility to trimethoprim (Tp) by an agar diffusion method. Seven-two (49.6%) strains (including E. coli, Proteus spp., E. cloacae, Citrobacter spp., Serratia spp., Klebsiella spp. and S. enteritidis) were considered as resistant to Tp, and 28.1% of the resistant strains could transfer the Tp-resistance marker into E. coli by conjugation. Tp-resistance was encoded by plasmids of different molecular weights, ranging from 52.5 to 165 kb (mean 107.5). These plasmids encoded a high level resistance to Tp (MIC greater-than-or-equal-to 1000 mug/ml). The other resistance markers co-transferred with the Tp were ampicillin (80.9%), sulfamethoxazole (61.9%), streptomycin (57.1%), kanamycin (47.6%), tetracycline (42.8%), tobramycin, gentamicin and chloramphenicol (28.5%, respectively). These data suggest that: (I) plasmid-mediated resistance to Tp represents a serious problem in the Canary Islands; (II) in at least 42.9% of the strains, resistance to Tp is not due to dissemination of Tn7, encoding joint-resistance to Tp and streptomycin; (III) selection of Tp-resistant strains may be achieved by unrelated antibiotics such ampicillin, whose resistance is encoded by the same plasmid.
Cent quarante-cinq souches d'Enterobacteriaceae isolées à l'Hôpital Nuestra Señora del Pino (Grande Canarie), pendant deux mois, ont été étudiées. Soixante douze souches (49,6 %), appartenant aux genres Escherichia, Proteus, Enterobacter, Citrobacter, Serratia, Klebsiella et Salmonella étaient considérées résistantes au triméthoprime (Tp). 28,1 % de ces souches ont été capables de transférer la résistance au Tp par conjugaison. Cette résistance était codée par des plasmides de poids moléculaires compris entre 52.5 et 165 kb. Ces plasmides codent un haut niveau de résistance au Tp (CMI ≥ 1000 μg/ml). Les autres résistances cotransférées avec le Tp étaient : ampicilline (80,9 %), sulfaméthoxazole (61,9 %), streptomycine (57,1 %), kanamycine (47,6 %), tétracycline (42,8 %), tobramycine, gentamicine et chloramphénicol respectivement (28,5 %). Ces résultats suggèrent que : (I) la résistance au Tp codée par des plasmides représente un important problème aux Iles Canaries ; (II) dans 42,9 % des cas, la résistance au Tp n'est pas due à la dissémination du transposon Tn7 ; (III) la sélection de souches Tp-résistantes peut être obtenue avec l'utilisation d'antibiotiques comme l'ampicilline quand cette résistance est codée par le même plasmide.
