Issue Date: | 2008 | Journal: | Dialisis y Trasplante | Abstract: | Aldosterone produces retention of Na+ and water and loss of K+, as well as substantial independent effects on epithelia. The use of mineralocorticoid receptor blockers reduces the damage produced by aldosterone in patients with cardio-renal dysfunction. Eplerenone, a novel and specific antagonist of mineralocorticoid receptors, has been approved for the treatment of patients with left ventricular systolic dysfunction after myocardial infarction. In this brief review, we analyze the effects of aldosterone, mineralocorticoid receptor blockade, and the molecule eplerenone, as well as the actions of this drug at different levels and its adverse effects. © Sociedad Española de Diálisis y Trasplante. La aldosterona produce retención de Na+ y agua y pérdida de K+, además de importantes acciones independientes sobre el epitelio. El uso de bloqueadores de los receptores mineralocorticoides reduce el daño que la aldosterona produce en sujetos con disfunción cardiorrenal. La eplerenona, un nuevo antagonista específico de los receptores mineralocorticoides ha sido aprobado para el tratamiento de pacientes con disfunción sistólica ventricular izquierda tras infarto de miocardio. En esta breve revisión analizamos los efectos de la aldosterona, el bloqueo de los receptores mineralocorticoides y la molécula eplerenona, sus acciones a diferentes niveles y efectos secundarios. |
