Title: Parathyroidectomy increases bone mineral density in primary hyperparathyroidism. A preliminary study

Abstract: Background. Although it is generally accepted that primary hyperparathyroidism (PHP) surgery increases bone mineral density (BMD), some studies have shown incomplete recovery of it. We present in this study the BMD changes we found in a group of women who were suffering from PHP one year after the surgical intervention and we compared these results with those found in a control group composed by women with PHP who did not receive surgery. Experimental design. An open, prospective study with follow-up of one year. Patients. 78 females diagnosed with PHP. They were classified into surgery group, composed of 55 patients who underwent surgery, and conservative group composed of 23 patients who had no surgery. Methods. Serum calcium and parathyroid hormone (PTH) were determined. A lateral dorsolumbar radiograph of the spine was performed. Bone mineral density (BMD) was measured by Dual X-ray Absorptiometry (DXA) in the lumbar spine and in the proximal femur. Results. Basal BMD were similar in both groups, but after one year of follow-up, increased in Surgery Group at every site were bone mass was measured, while patients in Conservative Group showed an increase of BMD in some sites but a decrease in others. Overall, the rate of change was higher in those patients who underwent surgery than in those who did not. Conclusions. Parathyroidectomy produces and improvement of BMD both in the lumbar spine and in the proximal femur after one year of follow-up.
