Abstract: Spontaneous pneumothorax is more frequent in young male patients. The smoking habit seems to play an important role in its genesis. Clinically, pain and dyspnea are the predominant symptoms. The chest x-ray shows retraction of centripetal lung towards the pulmonary hilum, producing a "lung stump" image. Minimum high pleurostomy and pleural aspiration puncture are the most commonly performed surgical procedures, although video-assisted thoracic surgery is considered a better treatment method since it makes it possible to identify and treat the cause radically, avoiding relapses. We present a case of a 28-year old male patient, smoker, who came to emergency department due to chest pain and dyspnea. In the physical examination, absence of right hemithorax vesicular murmur lung was detected. The chest x-ray showed right pneumothorax with total retraction of the lung. The patient was referred to a hospital.
El neumotórax espontáneo es más frecuente en pacientes jóvenes del sexo masculino. El hábito de fumar desempeña un papel importante en su génesis. Clínicamente el dolor y la disnea son los síntomas predominantes. En la radiografía del tórax se observa la retracción del pulmón centrípeta hacia el hilio pulmonar, ocasionando la imagen de “muñón pulmonar”. La pleurostomía mínima alta y la punción pleural con aspiración son los procedimientos quirúrgicos más realizados, aunque la cirugía torácica vídeo asistida es un mejor método de tratamiento puesto que permite identificar y tratar la causa de forma radical evitando las recidivas. Presentamos el caso de un paciente varón de 28 años, fumador, que acude a Urgencias por dolor torácico y disnea. En la exploración física se detecta una abolición del murmullo vesicular en el pulmón derecho y en la radiografía de tórax se aprecia neumotórax derecho, con retracción total de dicho pulmón. Se decide traslado a centro hospitalario.
